Sam and Ruby are with a demon possessed man and Sam asks the demon where Lilith is. The demon antagonizes Sam and Sam uses his powers to draw the demon out and back to Hell. Ruby asks how it felt and Sam says he feels good, no more headaches. Dean watches this interaction, confronts Sam and punches Ruby. Ruby chokes Dean, then Sam stops the fight. The guy is still alive, so Ruby takes him to the hospital.

Dean leaves and Sam follows. Dean tells Sam he apparently doesn't need him anymore since he can just fight demons with Ruby, then he punches Sam. Dean asks what other powers he has. Sam says he can just send demons to Hell with his mind. Dean says he can use a knife, but Sam points out that his method doesn't kill the possessed person. Dean tells Sam that he's heading down a slippery slope by developing his mysterious power. He says if he didn't know any better, he would hunt him, and so would other hunters. Sam reminds Dean that he hunted demons by himself while Dean was gone. Dean asks Sam why he lied to him about using his powers and why he had to find out from an angel. He tells Sam that Castiel told him to stop him or he would which means God doesn't want Sam to do what he's doing.

They get home and Sam gets a call from an old hunter friend Travis. He asks them to meet him in Missouri. While in the car, Dean tells Sam about their parents when they were young and about the events that lead to Sam getting demon blood in his mouth. Sam tells Dean that he's known about the yellow eyed demon blood for about a year, but didn't say anything.

Meanwhile, a man named Jack Montgomery (played by Dameon Clarke) is ravenously eating dinner with his wife Michelle (played by Joanne Kelly). He eats several steaks and says he can't explain his insatiable hunger. Jack goes into the bathroom to brush his teeth and his bones start moving around under his skin. He crumples in pain. Later, he raids the refrigerator scarfing down chicken and raw, bloody meat.

Sam and Dean meet up with Travis, who they haven't seen in ten years. Travis has a broken arm so he needs help tracking down a Roogaroo (or Rugaroo or Ruggaroo). He tells them that a Roogaroo is a rotten toothed demon with worm skin and a hunger for long pig (human flesh). He says that they start out normal, but once the Roogaroo feeds on human flesh, they change into the monster forever.

Travis explains that being a Roogaroo is genetic and he had once killed a Roogaroo man. Later, he found out that the man had a son who was adopted and spent years tracking him down. He finally found him, a man named Jack Montgomery. Travis says the only way to kill it is by burning the monster alive.

The guys prepare their torch weapons. Sam discovers that if the person hasn't fed on humans yet, they can stave off the change. Sam wants to talk to Sam, but Travis laughs at him, saying to Jack he's just meat on legs. Sam convinces them to let him talk to Jack and try to tell him what is happening to him before they kill him.

Michelle cuts her hand and when Jack sees the blood, he freaks out and leaves. He heads to a bar and while there, sees a man hitting on a woman. The woman isn't interested, but the man persists. Jack get up and tells the man to leave. The man throws a punch, but Jack grabs his fist and breaks his hand. Jack then returns home and sneaks up on his wife. She's mad at him for leaving suddenly, but he grabs her and starts kissing her. He starts getting a big more aggressive and Michelle pushes him off.

Sam and Dean head off to talk to Jack. Dean asks Sam if his emotions are getting in the way since maybe he can relate to something bad being inside someone good. Sam orders Dean to stop the car. He yells at him for being such a jerk and that's why he doesn't want to talk about his abilities. He tells Dean he's just trying to find a way to make something good come out of the demon blood in him. Dean asks Sam if he'll be able to make the kill if it becomes necessary.

Sam and Dean approach Jack in his yard. They tell him about what is happening to his body. He yells and tells them to leave before he calls the police. Later they follow Jack to a bus bench. Jack sees a woman undress in a window and gets the hunger. He scales the fire escape and Sam and Dean grab their fire torches and head up to the woman's apartment. Just as Jack is about to break in, the woman turns off the lights, making him see his reflection in the window. He changes his mind and leaves. Sam and Dean break down the woman's door. She screams and threatens to call the police. Sam and Dean leave.

Jack returns home to find Michelle tied to a chair. Travis comes up behind Jack and knocks him out. While Jack and Michelle are tied up, Travis explains that he has to kill both of them. Jack insists that he only has to kill him, but Travis gets Michelle to tell him she's pregnant. Travis starts to pour gasoline around the house, but Jack breaks free. He jumps on Travis, bites his neck, then continues to feed in front of his wife. Michelle is untied, runs to her car and drives off.

Sam and Dean arrive at the house and see the trail of blood leading to a dead Travis. Jack jumps out and fighting ensues. Jack gets a hold of one of the fire tanks and beats Sam with it. He then locks Sam in a closet.

When Sam comes to, he calls for Dean. Jack tells him he's still alive. Sam tries to talk to Jack while grabbing a wire coat hanger to pick the lock. Jack tells Sam Travis tried to burn him and Michelle alive. Sam asks why Travis would burn Michelle and Jack figures that they don't know about the baby. Jack doesn't say anything about the pregnancy. Sam breaks out of the closet and burns Jack alive.

On the way home, Dean tells Sam he did the right thing. Sam tells him he doesn't want to talk. Dean tells Sam that he's not alone and that he'll help him through whatever he's going through. Sam says he's decided not to use his powers anymore, but not because of him or the angel, but because it's his choice.

I'm not convinced that Sam is going to give up his powers. I think Ruby will talk him into pulling them out again, or he might need to use them to save Dean. I wonder how long it will be before the brothers find out about Jack's baby. That's some good foresight by the writers to ensure a future episode.
