Sam and Dean talk to the man who was shot. He says the doctors can't explain it. Dean asks how he explains it. The man says he thinks the lord is giving him a second chance. He says he has a feeling his arc angel is looking over him. Sam asks if he went to a crossroads or met anyone with black or red eyes. The man asks who they are and Sam and Dean decide to leave.

Sam tells Dean he talked to the cancer patient who came back to life after being clinically dead. Dean shows Sam a picture of a kid named Cole and says he was the last person who died in the town. Sam suggests that there is no local reaper to take the souls away. Sam says they should try to talk to Cole. Later that night, Sam and Dean go to Cole's grave. Sam lights some candles and places some herbs in a bowl. Dean says if they fix what's wrong, good people are going to drop dead. Sam says that there is a natural order that has to be obeyed. Dean reminds Sam that they escape death every day. Sam says they are an exception since he has demon blood in him and Dean has been to Hell and back. A man then arrives at the cemetery and asks the brothers what they are doing. He says it looks like devil worship. Sam tells the man they will leave. The man then says they aren't going anywhere ever again and calls Sam by his name. The brothers realize the man is Alistair. He then throws Dean across the cemetery. Sam uses his psychic ability and pins Alastair against a tree. Sam then pulls the demon from the man's body.

Sam and Dean return to their hotel room. Dean says he is in pain and asks what happened to Alastair. Sam says Alastair tried to fling him, but couldn't so he bailed. Dean tells Sam not to treat him like an idiot. Sam says he isn't keeping secrets, but Dean doesn't believe him. Sam says Bobby called them and thinks he is right about the local reaper. Sam reads a passage from Revelations. He says if someone kills a reaper it equals breaking a seal. Sam wonders how they are going to be able to find the reaper since they are invisible unless you are dead.

The brothers call Pamela over and says they want to take their souls out of their bodies and walk around the spirit world and look for the reaper. She tells them they're crazy. Dean says they are looking at the end of the world and they need her help. She asks how they plan on saving the reaper when they find it. She says they will just be clouds of fog. Dean says they'll figure it out. Pamela then reads the spell. Sam and Dean sit up, then see their bodies on the bed. Pamela tells them she will whisper the incantation in their ear for when they are ready to come back.

Ghost Sam and Dean go for a walk and a runner passes through them. Dean laughs and shoves his fist through Sam. They walk through the neighborhood and Sam sees a kid in a window. Dean recognizes the kid as Cole, the last person to die in the town.
