Izzy knocks on Meredith's door and a naked Derek answers. Izzy nervously asks for money to pay for a pizza she ordered and that Alex owes her $20, but he won't pay her back. Derek tells Izzy to hide, calls out to Alex, demands $20 which he gets, then gives it to Izzy. Derek then tells her not to bother him anymore.

Meredith returns home and finds Derek, naked in bed, reading a diary. She finds out that it's her mother's diary and she starts to get upset. He explains that he was just moving stuff out of the closet so he had room to move his clothes in and he just happened to notice the diary.

Erica asks Callie to go out on a date and Callie says yes.

At work, Dr. Webber reminds the staff that juniors don't specialize and reiterates the assignment shake ups to be doled out by Dr. Baily. Izzy and Alex are still teamed together, Meredith is sent to work with Dr. Erica Hahn, and Cristina is sent to work in the clinic. The assignments upset all but Meredith who is excited, but a little scared, to get to study with Dr. Hahn.

Dr. Webber tells George that his results from his intern test haven't come in yet, but he will let him know as soon as they do. Lexie tells George that she knows he passed and that she wants to take him out for a drink later to celebrate. He's not convinced that he passed, so she says if he didn't she'll still take him out for a drink.

Izzy and Alex find out that their first patient is a man with a gash on his head who simply needs stitches. Alex tells Izzy the patient is all hers. She goes to treat the wound and finds that it was caused by his wife who threw the remote at him out of frustration over his laziness. She tells Izzy about how they went to Europe for six months, but didn't see anything because he slept the whole time. Izzy cleans up his wound and notices his mouth twitch. She gets suspicious that there's a bigger problem and orders tests.

Meredith and Dr. Hahn are supposed to operate on a young boy named Duncan who has a heart condition. The boy is scared and takes off running through the hospital. He holds on to the fire alarm and threatens to pull unless Meredith will promise he won't have surgery. Meredith says she can't make that promise, so he pulls the alarm.

Meredith goes to Cristina for advice on how to work with Dr. Hahn. Cristina tells her to ask a specific question to illustrate that she has a grasp on the subject. Meredith takes her advice and impresses Dr. Hahn. She asks Meredith if she knows how to perform a specific task. Meredith lies and says she does.

Lexie tells Cristina that she has a patient in the clinic complaining about a rash. Cristina is annoyed at being sent to the boring cases. The woman sees Cristina and says she doesn't want her to be her doctor because Cristina is mean. When Cristina looks at the woman's rash, she trumps it up to be a flesh eating bacteria and calls Dr. Sloan to approve surgery. He shows up and tells her it's just a rash and sends her to fetch some cream from the dermatology area.

Alex gets the results back from the tests Izzy ordered on her patient and he sees that the patient has a large brain tumor. He takes the case to Dr. Shepherd and tells him that Izzy gave him the case to cheer him up. Meanwhile, Izzy goes to see her patient and he tells her that he has a new doctor.

Meredith and George try to show Duncan that having his IV inserted will only hurt a little by inserting one into George. He admits it hurts at first, but then it doesn't and Duncan allows him to put one in him. Duncan's parents tell George he's really good with Duncan.

Callie goes to Dr. Bailey and awkwardly tells her about being nervous about her first date with Erica because she isn't really familiar with a woman's down there area. Dr. Bailey gives her a horrified look and Callie asks her to forget she said anything.

Later, Dr. Bailey notices Dr. Hahn making eyes at Callie as she walks off into the elevator. Dr. Bailey then goes to Callie and using complex code, tells her about what to do with Erica. After lots of confusion, she finally tells Callie she needs to talk to Erica and find out what she needs to do.

Izzy goes to Dr. Shepherd and demands to get her patient back. He tells her that Alex is on the case now and she'll have to wait until next time. Dr. Sloan tries to advise Izzy that surgery is like the old west and Alex is a cowboy. Izzy finds Alex and yells at him for stealing her patient. He is unsympathetic.

Cristina goes to the Dermatology room and asks for the cream for her patients. The woman at the desk asks her if she wants some raspberry water and Cristina says sure. The woman then gets a hand massage and explains that doctors hands are very important so they all get daily massages. Cristina finds out that the woman is a resident doctor. She looks around and sees the room is full of happy, calm, and well taken care of doctors.

Cristina calls Meredith to meet her in her newly discovered sanctuary. Meredith arrives and Cristina tells her all the happy people are actually doctors. They wonder how they can be so peaceful and well rested. Meredith tells Cristina she lied to Dr. Hahn about knowing how to stitch an artery. Cristina tells her to bring her some grapes and shows her how.

Izzy then show up to Dermatology and discovers what Cristina and Meredith are experiencing. The three sit on the couch in awe. George shows up and tries to talk sense into the doctors by saying that the dermatologists are happy because they work with lotion. They wonder if they would be happier if they loved lotion instead of cutting people up and eventually conclude they like surgery. George then tells Izzy that she should consider using her other skills to get her surgery back.
